Preparticipation Physical Evaluation

Many Sports Leagues require children to have completed a Physical Exam before Participation

The overall goal in performing a preparticipation physical evaluation also known as PPE, is to promote the health and safety of athletes in training and in competition. Sports Physicals provide the medical background on which physical activity decisions will be made by the Family Physician. Most schools require an updated physical prior to the start of the school year (and some, even before sports tryouts). Additionally, many doctor's offices become very busy at the start of the school year. The sooner you get your child's physical, the less time it will take from your already busy schedule!

What to bring to Child's Physical

  • A copy of the entire sports physical form for the provider to review and sign
  • Your child's prescription eye glasses or contacts if they wear them
  • If you are a New Patient: You will need a copy of your child's latest immunizations
