New Patients


All well exams are scheduled first thing in the morning. (You must be fasting, nothing to eat or drink after midnight the night before.) If you are coming for immunizations, please let the receptionist know if you are expecting more than one injection. Flu, Testosterone, and B-12 injections do not require an appointment, but please call in advance to make sure we have the medication on hand.



If you need a prescription refilled, please contact your pharmacy and have them fax us a refill request. If you require a change in medication or dosage, you must make an appointment to see the physician.

All prescriptions will be called in, faxed back, or written by the end of every day except for Wednesdays.

Please bring a list of your medications to every appointment, even if you have been recently been seen. In addition, update the list if a new medication is added or you have stopped taking anything. This includes aspirin and supplements.


The laws have changed and we are no longer able to fax them to the pharmacy. You will have to call the office and Iet the receptionist know that you need a prescription. Please make sure to leave your full name and date of birth. You may pick up your prescription/s at the end of the day in our office. In addition, if you are getting controlled substances from other physicians, Dr. Simons will not fill your prescription. NO EXCEPTIONS!!! You must see the doctor every 3 months for all controlled substance prescriptions.


Please call your mail order pharmacy and have them fax us a refill request. We will fax the prescription back to them before the end of the day.


All paperwork that needs to be filled out by the doctor can be done by appointment or can be dropped off for a $25.00 fee. 


If you need a referral to a specialist, please make an appointment with Dr. Simons first. He will discuss your need for the referral and will send you to the closest specialist available with whom he is confident will give you excellent care. If needed, we can provide you a list of quality specialists that Dr. Simons commonly refers to.
